NEW STUDENT ENROLLMENT INFORMATION
All new students must sign up for a date and time for a phone appointment on the link below.
Once an appointment has been scheduled, our Attendance Operator, Cindy Gingery, will send you an Enrollment Form to be completed and returned before your appointment. You will need to attach and send the following documents and proofs back with the Enrollment Form.
Certified Birth Certificate(not mother's copy)
Social Security Card
TN Immunization Record w/ proof of physical
Two Proofs of Residency from the list below
Acceptable Proofs of Residency
Most recent MLG&W bill (no cut-off notices or Bartlett water bills)
Current mortgage statement or deed of the homeowner
Lease of the lessee of the home
Rental agreement of the renter of the home
Current real-estate tax receipt of the homeowner
Current public assistance/government benefits check, card, or papers
